Complaints can be submitted to the Village for review in a couple of different ways.

Call the Village Hall at 815-335-2020 and report your complaint
Fill out the complaint form PDF on this page and return it to the Village Hall
Fill out the webform complaint form on this page and click submit.

While it is helpful for us to have your contact information in case there is need for further information, your personal information is not required to submit a complaint.
